Amlodipine is a blocker of "slow" calcium channels of the group of dihydropyridine derivatives of the second generation. It works mainly on blood vessels, causing vasodilatation of smooth muscle and vessel walls. It also lowers blood pressure and reduces peripheral vascular resistance.
Amlodipine does not allow the entry of calcium ions into the smooth muscle cells of the myocardium and blood vessels. The drug produces a prolonged dose-dependent hypotensive effect, thanks to the direct relaxing effect on vascular smooth muscle, decreasing their total peripheral resistance. Due to expansion of peripheral arterioles and reducing systemic vascular resistance it reduces the severity of myocardial ischemia almost without changes in heart rate. Since there is a lowering of the heart load, myocardial oxygen demand is reduced, the major coronary arteries and arterioles dilate both in unmodified and ischemic areas of the heart muscle increase the supply of oxygen to the myocardium.
Amlodipine gradually manifests its action and prolonged effect. That is why there is a slow decrease in blood pressure and minimum reflex stimulation of the sympathetic nervous system. Amlodipine reduces the degree of left ventricular hypertrophy in hypertension. Significant reduction in blood pressure is observed after 6-10 h after administration of amlodipine. The duration of the effect is 24 h. During prolonged treatment the maximum reduction in blood pressure occurs within 6-12 hours after administration of amlodipine. If after prolonged treatment amlodipine is canceled - the effective blood pressure reduction is retained for 48 hours after the last dose. Then the BP readings gradually return to baseline within 5-6 days.
Amlodipine gives angina patients an opportunity to increase run time of exercise, inhibits the development of angina attacks and St-segment depression during exercise, the frequency of angina attacks significantly reduces as the number of nitroglycerin tablets.
In patients with heart failure of II-III functional class Amlodipine does not impair the state.
Amlodipine may be used in patients with diabetes, gout, bronchial asthma. The drug has no effect on the lipid profile of blood plasma and the metabolism.

Amlodipine is indicated for the treatment of hypertension as a means of first-line drug used for monotherapy of hypertension in many patients. Amlodipine can be simultaneously used in combination with a thiazide diuretic, РћВ±-adrenergic-antagonists (РћВ±-blockers), РћР†-adrenergic-antagonists (РћР†-blockers) and ACE inhibitors.
